Life Steps Foundation Southern California Adult Services, known as LSF SoCal Adult, is a committed organization that serves more than three hundred participants across the Southern California region. Our purpose is to aid adults of all ages who face disability-related challenges, working to enhance their independence, address their personal needs and preferences, and support them in preserving a satisfying quality of life. We are presently looking for a caring and devoted Community Support Facilitator to join our team in a part-time capacity within the San Gabriel Valley area.

Responsibilities

  • Deliver personal care services to clients, such as preparing meals, helping with eating, bathing, using the toilet, and general physical activity.
  • Carry out light cleaning tasks as needed.
  • Observe and help with medication management.
  • Offer on-call support to clients during illness or emergencies.
  • Assist individuals with wheelchair use and other mobility requirements.
  • Recognize natural and community supports and help the individual access them.
  • Support individuals in building and sustaining relationships.
  • Aid in locating suitable roommates and housing.
  • Submit accurate, complete, and readable billing and payroll documents promptly.
  • Follow the approved monthly schedule and complete authorized service hours.

Qualifications

  • High School Diploma or GED is required.
  • Previous experience in case management or as a personal attendant.
  • Reliable transportation is essential.
  • Valid Driver\'s License and automobile insurance.
