Office RN/LVN Experienced in Home Health Care (Valley Village)

An immediate opportunity is available for an experienced Home Health Registered Nurse or Licensed Vocational Nurse. This position is based primarily at our North Hollywood office, with occasional, infrequent visits to patient residences. The role focuses heavily on office duties, including telephone triage and assisting with the completion of 485s and Plans of Treatment using the computerized Soneto program, which codes and types care plans. Responsibilities also involve reviewing nursing and multidisciplinary notes, as well as Evaluations and Recertifications for a non-Medicare Home Health Agency (OASIS not required). The caseload consists mainly of private duty patients.
This is a full-time role. Health benefits are available after a three-month training period, and a 401K plan is offered after one year of service. The standard hours are Monday through Friday, 10:00 AM to 7:00 PM. There may be occasional needs to work a 7:00 AM to 4:00 PM shift.
This is strictly an office-based position. The hourly rate is $30 to $45 for LVNs and $60 to $75 for RNs.
Position Responsibilities
- Conduct initial patient intake and verify insurance; provide clinical information to field nurses.
- Review clinical progress notes submitted by field staff.
- Deliver orientations and in-service training to all field staff.
- Perform telephone triage for new intakes, order medical supplies, and coordinate patient care.
- Maintain the Infection Control Log and draft incident reports.
- Engage in Quality Assurance and Quality Improvement activities, including performing quality management chart reviews.
- Participate in QA/QI meetings.
- Share after-hours telephone triage duties with existing management staff.
- Perform office work supporting efforts toward ACHC certification.
- Collaborate with existing Supervisory RNs.
We seek a positive, upbeat individual who is patient and detail-oriented. The ideal candidate will communicate respectfully with both management and field staff. A \"people person\" who can dedicate time to encourage field staff in person and over the phone—using praise to foster success—is preferred. The role requires patience to work with both skilled private duty staff and non-skilled personnel, including companions, CNAs, and CHHAs. The candidate must collaborate with the staffing coordinator and clerical staff to oversee basic activities and possess strong computer skills.
