Our social services agency is looking for a committed and understanding Case Manager to assist adults with developmental disabilities. This full-time professional role involves coordinating services, creating customized plans, and acting as a primary advocate for clients pursuing increased independence.

Primary Duties

  • Examine and interpret assessment information to formulate thorough Individual Service Plans (ISPs).
  • Organize and participate in meetings with interdisciplinary teams.
  • Generate Semi-Annual Progress Reports and ensure case files are kept accurate.
  • Instruct and assist direct care staff with implementing objectives and maintaining records.
  • Offer job coaching assistance as required.
  • Guarantee all records adhere to confidentiality policies and regulatory criteria.
  • Track client advancement and modify services to address changing needs.
  • Act as a connection point between clients, their families, and service providers.
  • Conduct necessary field visits with clients.
  • Undertake additional duties as assigned.

Required Qualifications

  • A Bachelor\'s degree is necessary, with preference given to Human Services or related fields.
  • Two years of prior experience creating and executing vocational plans in a program environment is preferred.
  • Background in devising and applying behavior modification intervention services is advantageous.
  • Direct experience supporting individuals with developmental disabilities.
  • CPR and First Aid certification (must be obtained within 30 days of starting).
  • A clear criminal history check, following DDS licensing stipulations (LIC 508).
  • Access to dependable transportation and a valid California driver\'s license.
  • Current automobile insurance meeting state minimums.
  • Firm understanding of behavior modification principles and human development.
  • Superior organizational skills and the capacity to work autonomously.
  • Exceptional English written and spoken communication skills (Spanish language skills are a bonus).
  • Proven ability to collaborate productively within an interdisciplinary team.
