The hourly wage for this role is between $44.64 and $58.15. Compensation is aligned with experience.
POSITION SUMMARY
Reporting to the Nursing Supervisor, the Registered Nurse will deliver culturally sensitive medical health services to low-income residents of Los Angeles County, with a particular emphasis on the LGBT community.
The Registered Nurse will ensure the provision of quality, cost-effective care for clinic patients, manage patient treatment plans, and coordinate with providers and other clinical support staff to achieve positive patient outcomes within set timeframes and with efficient resource use. Furthermore, the Registered Nurse will collaborate closely with the Medical Director and Nursing Supervisor to design, implement, and monitor workflow processes across all APLA Health locations related to patient-centered medical home (PCMH) implementation. This is primarily a clinical role.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Offer clinical support to medical providers before and after primary care visits to facilitate the prompt delivery of quality medical care.
- Coordinate with medical assistants to ensure patients receive specialty referrals promptly and can follow through without interrupting care continuity.
- Treat patients diagnosed with sexually transmitted infections (STIs) promptly and in accordance with APLAHW clinical STI treatment guidelines.
- Administer vaccines to patients per APLAHW clinical guidelines.
- Provide and properly document health education for patients.
- Take part in quality improvement activities as directed by the medical director, such as data analysis and outcome measurement, documenting and reporting the results of these initiatives.
- Triage patients who walk into the clinic or call with acute medical concerns.
- Assist front office staff in making suitable arrangements for patients who arrive late or need rescheduled appointments by reviewing medical records and determining the urgency and location for the next visit.
- Provide guidance to the clinic LVN and MAs by supervising front and back office staff to maintain efficiency, professionalism, and courteous patient interactions. This may include serving as backup when needed.
- Assist in the clinic\'s transition to a patient-centered medical home (PCMH) in collaboration with the PCMH coordinator, medical director, chief operating officer, and clinical nurse manager.
- Maintain sufficient medical supplies and medications for clinical areas by monitoring inventory and placing orders as necessary. Ensure exam rooms are fully stocked and prepared for patient care.
- Participate in chronic disease management for selected patients with special needs.
- Communicate laboratory results to patients.
- Assist patients in connecting to APLA services such as benefits counseling, mental health, case management, and dental care.
- Serve as the initial point of contact for the eCW patient portal, responding within scope when appropriate and forwarding inquiries to the relevant provider when indicated.
- Obtain prior authorizations for medications.
- Contact insurance carriers to verify medication eligibility and requirements.
- Request, track, and secure prior authorizations from insurance carriers.
- Communicate any insurance changes or trends to the team.
- Clearly document all communications and contacts with insurance companies, Pharmacy Benefit Organizations (PBOs), and pharmacies.
Other duties may be assigned as needed to meet business requirements.
REQUIREMENTS
Training and Experience:
A valid California Registered Nurse (RN) license issued by the California Board of Registered Nursing (BRN). Current Basic Cardiac Life Support (BCLS) and Cardiopulmonary Resuscitation (CPR) certification. Clinical experience as a registered nurse in an ambulatory healthcare clinic, preferably at a federally qualified health center (FQHC). Experience working in patient-centered care teams is preferred.
Must possess problem-solving and organizational skills to ensure a productive work environment and goal achievement. Experience in chronic disease management, case management, utilization management, and quality improvement projects is preferred. Experience in an HIV medical practice is also preferred.
Knowledge of:
Knowledge and experience working with electronic health records, with a preference for Eclinical Works.
Ability to:
Ability to deliver services in a non-judgmental manner and work effectively with diverse populations is required, as is the ability to maintain records and follow clinical guidelines and protocols. Must be able to work efficiently and complete tasks with a high degree of accuracy, work and solve problems independently, and work flexible hours to complete tasks and meet client needs. The ability to be flexible in handling unanticipated client needs is required.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS
This is primarily an office-based position requiring only occasional bending, reaching, stooping, lifting, and moving of office materials weighing 25 pounds or less. The role necessitates daily use of a personal computer for entering, viewing, and revising text and graphics both on-screen and on paper.
SPECIAL REQUIREMENTS
Must possess a valid California driver’s license, proof of automobile liability insurance, and have access to a personal vehicle for work-related purposes. COVID-19 Vaccination and booster or a valid Medical/Religious Exemption is required.
