Position Overview

This is a full-time position requiring 40 hours per week.
Hourly Rate: $26
Work Location: In-person

The Case Manager will deliver services in the field, at client homes, and/or in an office setting. Responsibilities include conducting assessments, creating treatment plans, providing individual, group, and family therapy, managing discharge planning, and participating in Child and Family Team meetings.

Although this role is based from our Glendale office, it is primarily field-based, serving clients across Los Angeles Service Planning Area 2. This region encompasses, but is not limited to: Calabasas, Glendale, San Fernando, Santa Clarita, Agoura Hills, Castaic, Chatsworth, Northridge, Pacoima, Reseda, Saugus, Sherman Oaks, Sunland, Tujunga, and Van Nuys.

Job Duties and Responsibilities

  • Manage and supervise a caseload of 23 families or clients, facilitating scheduled monthly in-person home visits that align with client and family needs.
  • Respond to and manage crisis situations as they arise during work hours, and when appropriate, outside of standard working hours.
  • Perform intake assessments to gather information required for determining suitable and available services for the family.
  • Collaborate with the family to develop a written Individualized Service Plan (ISP).
  • Oversee the implementation, progress, and completion of the ISP.
  • Ensure referral linkages are finalized and accessible to the client, and provide additional community resources to support family health and stability.
  • Develop and maintain contacts and relationships with external agencies and individuals relevant to client cases.
  • Provide intensive home-based mental health services when determined appropriate by the treatment team.
  • Attend client-focused meetings as needed, such as IEP meetings, Wrap Around meetings, Child and Family Teams, target behavioral specialist meetings, and psychiatry appointments.
  • Assist in coordinating special events, services, or supports as required.
  • Attend, facilitate, or co-lead support groups upon request.

Required Qualifications

  • Bilingual proficiency in English and Spanish.
  • A bachelor\'s degree from an accredited institution in one of the following fields: social work; marriage, family and child counseling; child psychology; child development; counseling psychology; social psychology; clinical psychology; educational psychology; or education with a counseling emphasis.
  • Experience working within child welfare and family systems, and/or in trauma-informed practice.
  • Adoption-related experience is preferred.
  • Must obtain and maintain a clear criminal record free of disqualifying convictions.
  • Must possess and maintain a valid California Driver License and a driving record that meets the standards of Olive Crest\'s automobile insurance policy.
  • Working knowledge of the Microsoft Office Suite.
